You may complete applications for new P&C and A&S business online or download applications.
Placing Quoted Business:
A New Broker Kit must be completed and returned to the Licensing Department before placing your first piece of business in any of our programs. In some states, carrier appointment is also required.
If you have never placed business with us before, click here to complete and submit an Agency Questionnaire online. Or, print the Agency Questionnaire and return the completed form either by facsimile attention to: Licensing Dept. 717-747-7008 or by mail to:

Upon receipt of the Agency Questionnaire, our Licensing Department will forward a New Broker Kit to your attention for completion. A kit may also be requested from our Licensing Department by simply calling 800-233-1957.
